- Bridge the gap between clients and the health/social service systems to assist in navigating and connecting to the resources needed to improve each family’s quality of life.
- Facilitates, moves and assists families in obtaining needs and asks as applicable to each child in the program.
- Completes psychosocial assessments within agreed-upon timelines and follows the department process of shared plans of care and documentation of progress toward goals as applicable.
- Builds relationships with families by listening, identifying barriers, problem-solving and assisting them to develop more effective systems of care for their child.
- Attends educational seminars and relevant conferences as needed to discuss program needs/issues.
- Provides support and information to improve the quality of life for children and families.
- Assists families with the psychosocial aspects of transitioning to Transitional Care as applicable to assigned area(s).
- Proactively utilizes testing/screening tools, processes to make referrals to community agencies, and assists with completion of applications as appropriate. Develops, maintains, and utilizes a current list of community resources.
- Interviews and compiles information to complete social histories for families in the program or onboarding into the program.
- Provides education related to cultural diversity, abuse, neglect, client rights and responsibilities, effective problem-solving, confl ict resolution, community health and social services available as applicable to each family.
- Supports greater Montana Pediatrics Care Coordination team by communicating identified issues related to Social Determinants of Health (SDOH) in a proactive and professional manner. Collaborates with appropriate stakeholders for optimal care and services coordination.
- Assists families in completing forms and gathering necessary supporting documentation. Follows up to ensure notices and/or requests for information are responded to in a timely manner. Follows- up with the application agency and the family through completion and advocates on behalf of a child as applicable.
- Encourages and assists with obtaining a primary care provider, specialists, and other providers and professionals as needed for each child and family to form a robust and appropriate care team.
- This role is usually filled by an individual with a Bachelor’s Degree in Social Work, however, we are open to individuals who have Bachelor’s Degree in a related field, relevant work experience, and a passion for solving healthcare issues for families with medical complexity.
- 2+ years of professional or personal experience in a clinical or social support role is preferred.
- Willingness to participate in additional training opportunities as applicable for the role requirements and/or as needed.
- The drive to improve the healthcare delivery systems through innovative and out-of-the-box solutions.
- Excellent interpersonal skills with the ability to work across multiple healthcare disciplines both internally and externally with stakeholders.
- Excellent organizational skills, detail orientation, a self-starting attitude, and critical thinking skills.
- Our models of care continuously engage in how technology can improve the patient experience and make healthcare more accessible to more individuals, as such, knowledge and understanding of how to utilize and navigate different technology, open to exploring new technology, comfortable and willing to engage in telemedicine, and excellent at utilizing software platforms such as Microsoft; Excel; and Google.
- Basic understanding of EMR platforms; experience a bonus.
- Basic understanding of project management platforms; experience a bonus.

- This position is part-time, up to 28 hours per week.
- Montana Pediatrics has an office location in Kalispell, however, telecommuting can be available for the right candidate.
Positions funded by grants are typically of limited duration and are subject to immediate termination if the grant funding ceases. In addition, at Montana Pediatrics discretion, employment may be terminated before the completion of the terms of the grant. Alternatively, grant-funded positions may extend past the grant expiration should the duties and responsibilities of that position need to be retained to fulfill the strategic goals of Montana Pediatrics.
