Care coordination and care management is essential to improving the lives of our families and children in Montana, especially for those with medical complexity or special healthcare needs.
We are excited to participate in a unique learning opportunity for a multidisciplinary team in Montana. Through a partnership with the University of Montana’s Rural Institute, our participants include family members, administrators and practitioners. The Care Coordination Academy’s educational series is aimed at growing the knowledge and skills of the participants around effective implementation and evaluation of care coordination. We hope what we learn here will be part of building toward an effective and sustainable model of care coordination for Montana’s children.
